PBLC Site Information & Documentation
SITE FILE FORMS
The following forms include information which each site must keep on file throughout the program year:
AmeriCorps Provision, (B)(3)(b) requires all organizations sponsoring PBLC project sites to consult with and obtain written concurrence from any site-based unions or nonsite-based unions whose employees perform similar work as that to be performed by your AmeriCorps Members in the same community. Please complete this form at the outset of the program year and keep it in your PBLC files.
AmeriCorps Provision, (B)(6)(h) requires programs with members or employees who have substantial direct contact with children (as defined by state law) or who perform service in the homes of children to conduct criminal reference checks on these members or employees as part of the screening process. Please complete this form at the outset of the program year and keep it in your PBLC files. If you determine that this provision does apply to your project, please maintain documentation that the required criminal reference checks were conducted in your files.
Member Safety Certification Form
AmeriCorps Provision, (C)(29) requires programs to institute safeguards as necessary and appropriate to ensure the safety of members. Furthermore, it prohibits sites from forcing members to participate in projects that pose undue safety risks. Please complete this certification form at the outset of the program year and keep it in your PBLC files.

Included are the current contact information for and descriptions of the various PBLC projects for your use and reference throughout the year. Information is subject to change throughout the program year and updates will be sent out as necessary.

This document provides sites with a brief overview of the purpose of a site visit, how they are conducted and what sites can expect if they are selected for a site visit by Equal Justice Works staff.
