Documents, Forms and Resources
Below are links to the important documents and forms for the 2008 Summer Corps program. As a Summer Corps member, you are required to submit all enrollment and exit materials in order to complete the program and receive a $1,000 AmeriCorps education award voucher.

Member Orientation Guide (337 KB)
This document provides you with information about Equal Justice Works, AmeriCorps and the Summer Corps program; requirements for participation in the program; a member checklist; and important deadlines and reporting instructions.
Supervisor Guide (267 KB)
This document describes the roles and responsibilities of a Summer Corps supervisor and an AmeriCorps host organization. Equal Justice Works has sent this guide to the supervisor you listed on your Summer Corps application.
All enrollment paperwork must be submitted to Equal Justice Works by the first day of your project or May 23, 2008, whichever date is earlier. The date of submission is considered to be the postmark date on your mailed enrollment form and service contract. We require you to send us the original copies of each document. Please be sure to make a copy for your files and consider a mode of mailing that provides delivery confirmation.
Member Enrollment Form (248 KB)
The submission of this form, including the signed drug-free workplace page and proof of AmeriCorps eligibility such as a copy of your unexpired U.S. passport, birth certificate, certificate of naturalization or Lawful Permanent Resident Card, is required of all AmeriCorps members by the beginning of their service.
Service Contract (297 KB)
This contract must be completed, including a position description, and signed by both you and your supervisor (or other official representative of your host organization) before you can be enrolled in the program and begin counting time toward your 300-hour requirement.
All exit paperwork must be submitted to Equal Justice Works within 14 days of the completion of your service or by Sept. 15, whichever date is earlier. Please be sure to keep a copy of your exit materials for your files and consider a mode of mailing that provides delivery confirmation.
Time Log (224 KB)
You will need to submit a completed time log for your entire term of service. All time logs must be signed by both you and your supervisor. Your service hours must total at least 300 for you to qualify for an education award. Keep track of your hours as you go!
Member Exit Form (227 KB)
The submission of this form, including the certification of non-participation in activities prohibited under AmeriCorps, is required for you to exit the program and receive a $1,000 education award voucher.
Online Final Report
The purpose of this report is to measure the results of your hard work and the success of the Summer Corps program as a whole. You will receive a link to the online report in July. In order to prepare the report, you will need to review the 2008 Summer Corps reporting instructions, which can be found in the Member Orientation Guide. Activity tracking forms are provided below to aid you in the completion of your report.
You do not need to submit these forms to Equal Justice Works, but we strongly recommend that you use them to keep track of your project activities throughout the summer. The Online Final Report will ask you for specific data such as the numbers of clients you served, resources you created and organizations you worked with, and having this information prepared in advance will make the reporting process much easier for you and help to ensure the accuracy of your data.
